Appointments
Please call in at the surgery or telephone 0113 2535134 during opening hours to arrange an appointment or use PATCHS . Any requests via PATCHS will be triaged and an appointment booked and confirmation sent to you OR you will be signposted to an appropriate service. All consultations will be for a 10 minute period. With the exception of Friday evenings all appointments are pre-bookable for 4 weeks in advance.
If you have a GP preference then an appointment will be allocated to the next available slot for that GP. Please bear in mind that a request of this nature may lead to a delay in appointment allocation.
When the GP's run behind it is inconvenient for our patients and stressful for the GP's.
Please arrive on time for you appointment
Please let us know if you cannot attend your appointment.
If you are late, then please rebook
Remember when you are with the doctor, once 10 minutes have passed, your appointment time is over and you will need to make another appointment
to deal with any further problems .

New Physiotherapy Service
Do you have aches and pains in 2 areas or less, without a fever or feeling unwell and not already under the care of MSK, orthopaedics, rheumatology and have had the aches and pains for less than 8 weeks ??? ( Please see Physio First Tab for full assessment criteria) If so, then you may be eligible for a free physiotherapy assessment without the need to see your GP.

Telephone Appointments
In order to fit in with the busy lives of our patients we do offer pre-bookable telephone consultations. If you feel that your matter could be dealt with in this way rather than attending the surgery please inform reception and a convenient appointment can be made.
Telephone consultations can be arranged to discuss results and ongoing medical certificate issues along with general medical advice. If during the conversation the doctor feels you need to be seen in person then a face-to-face appointment will be arranged.
On-Line Appointments
We are now offering appointmets on-line. To register for this service please call in to the surgery with photographic identification to receive your username and password. You will then be directed to https://systmonline.tpp-uk.com to use the new service.
Failure to Attend Appointments
Wasted appointments are a huge and costly problem for the NHS. We are constantly monitoring DNA (Did not attend) appointments which is very time consuming for us and missed appointments lead to a shortage of appointments. Some patients have a genuine reason not to attend but forget to cancel. You can cancel by texting CANCEL to the number on the confirmation text sent to you or ring us on 0113 2535134 at less busy times or you can call in. Appointments need to be cancelled in good time, preferably 24 hours before. In extreme circumstances we will accept a cancellation up to 1 hour before the appointment time (at our discretion).
Please be mindful that if you cancel your appointment in good time then another patient can benefit from that appointment.
Patients who consistently miss appointments without informing Gildersome Health Centre will receive a verbal or written reminder that they need to cancel in good time in the future and that if they continue to miss appointments they may be removed from the practice. If the patient then continues to miss appointments, they will receive a final warning. If they still abuse the appointment system, they will be advised they need to register elsewhere as this will be classed as a GP /Patient relationship breakdown.
